
MSN HomeCare offers a variety of family centered care options. Our home health care professionals include both skilled and non-skilled pediatric and adult caregivers. They are experienced with both pediatric and adult patients. Our nurses are fully licensed and all our care givers receive clinical support and supervision 24 hours a day, 7 days a week.
If you are a Facility Case Manager looking for provider options during the discharge process, MSN HomeCare offers two divisions:
- Provider Services
- individual on-going Case Management
Your client may be interested in medical or non-medical providers in the home and request a list of recommended companies during the discharge planning process. If so, MSN HomeCare will work with you and your client to determine the most appropriate caregiver based on the needs of the patient while working closely with you, the physician/care team, and even family members to determine the types of support needed for home services. Once the level of need is determined, a MSN HomeCare RN will provide an in-home assessment as well as a clinical assessment to further support definition of services in the home. The RN will then arrange for the appropriate staff to begin care as soon as possible for both client and agency. If skilled care continues for a period of time, an assessment/supervisory visit will be repeated every 60 days by a RN with RN on-call access 24hrs a day while MSN HomeCare provides services in the home. This type of referral is made simply by contacting your local MSN HomeCare office and you will be connected to our intake department or RN Case Manager who can discuss the details of the discharge and begin the process by with a Fax transmittal of the appropriate documents. The agency RN Case Manager is available to discuss any questions, concerns initially and an appointment for the home assessment is scheduled. In addition, families sometimes request an in-hospital introduction of the agency and if so, that can be arranged as well.
If only Care Management services are needed, arrangements are made with the referral source for definition of the service needs and then a clinical assessment of the client is done by the agency RN Case Manager in congress with recommendations from hospital case manager, physician/care team, and family members when appropriate. Care Management services include a variety of supports /services including assistance with the scheduling of or accompanying to Dr. appointments ,patient advocacy, assessments for changing levels of care, the arranging of other support services or financial assistance such as Medicare benefits, OT,PT, and Speech therapies and special monitoring in times of grief or post surgically. The RN Case Manger providing Care Management services is also available 24 hrs on-call.
MSN Home Care offers flexible options, from four hours (whenever the patient needs them to complete) to round the clock, 24-hour private duty care. MSN Home Care also offers high-tech services, with a respiratory therapist on staff.
